Bring your form(s) to:
- your school counselor if you have a 504 plan or
- to your IEP Case Manager if you have an IEP or
- ESOL teacher if your are in our ELL program

To request accomodations on the PSAT, PSAT/NMSQT, AP exams or on the SAT, have your parent complete the Parent Consent Form available on the College Board website.Then bring this form to your school counselor if you have a 504 plan or to your IEP Case Manager if you have an IEP or ESOL teacher if your are in our ELL program. Requests for the PSAT and SAT should be made 8-10 weeks in advance of your test date. Requests for accomodations for AP exams in May must be received by no later than December.
Calendar Specific Deadlines for College Board accomodations requests
If you have previously been approved by College Board for testing accommodations (for example, when you took the PSAT/NMSQT or SAT), you do not need to submit a new request.
Once your accommodations are approved by College Board, they remain in effect until one year after high school graduation (with some limited exceptions) and can be used on the SAT, SAT Subject Tests, PSAT/NMSQT, PSAT 10, and AP Exams. You do not need to request accommodations from SSD a second time.

All requests for accommodations and EL supports, including appeals, must be submitted by the published late registration deadline for your preferred test date through the Test Accessibility and Accommodations (TAA) system.

Register for your specified test date, complete the accommodations request form, submit this form to your school official atleast 4 weeks in advance of your test date.

If a student has been receiving and utilizing accommodations throughout high school, they may apply for IB testing accommodations. Testing accommodations must be officially requested from IB following IB regulations. The final decision granting accommodations is solely up to the IB organization. In order to request testing accommodations for IB exams (referred to as inclusive assessment arrangements), students should speak with our IB Coordinator/Mr. Imhof to inquire.
Accommodations for students at College with high school IEP's / 504's
Unknowingly, often students and parents believe that a student's IEP or 504 plan accommodations transfer to the post-secondary institution (vocational school, state college, university etc). However accomodations do not seamlessly transfer. Students must reach out to the specified institutions Office of Disabilities to inquire on the process for requesting accomodations at the college. Also, accomodations at post-secondary institutions are implemented differently than in high school. This worksheet provides a brief overview of how approved accomdations may look different at college compared to what you are used to.

Dual enrollment students taking the PERT test at PHSC can request accommodations through Student Accessibility Services.
For a quiet room (ADA room), students can directly contact the PHSC Testing Center or Academic Success Center.
Students taking courses via PHSC's online platform (Canvas), hybrid courses (partly on campus or Zoom and partly online), or fully face-to-face on a PHSC campus can receive accommodations through Student Accessibility Services at PHSC. Students taking courses through Hernando e-school (if applicable) or at SHS should use their accommodations as outlined in their IEP/504 plan.
The PERT test is untimed, so no additional testing time is needed. However, it is recommended to take different parts of the PERT on different days to manage time effectively. Students can schedule different days for the test when they sign up.
